What is premature ejaculation?

Premature ejaculation signifies the uncontrolled ejaculation either before or shortly after the sexual penetration. It occurs with minimal sexual stimulation and before the person wishes. This may result in unsatisfactory sex for both partners. It can increase the anxiety which may add to the problem.

The premature ejaculation is considered as one of the most common forms of male sexual dysfunction that has probably affected every man at some point in his life.

How many men experience premature ejaculation?

As per the information in some surveys, the prevalence of Premature Ejaculation in men somewhere between 15 to 30 percent. However, the prevalence of medically diagnosed and diagnosable PE is quite lower.

In an analysis of around 5,000 men in nine Asia-Pacific countries, 16 percent men met the criteria for a diagnosis of PE on the five-question Premature Ejaculation Diagnostic Tool (PEDT). Around 15% probable PE was found in respondents, while 13% men self-reported PE.

Types of Premature Ejaculation

There are two different types of premature ejaculation:

  • Primary or lifelong
  • Secondary or acquired

Couple Lifelong/Primary premature ejaculation is when a man does not have control of ejaculation from the time of his first sexual experience and it has been a persisting issue. When left untreated, this will continue for the rest of his life.

Usually, an acquired or secondary premature ejaculation is when there has been a period of normal functioning before the premature ejaculation started. It can arise due to medical conditions such as erectile dysfunction, diabetes, overactive thyroid and chronic prostatis. It may also be caused due to the psychological issues such as stress, anxiety, relationship problems, infrequent sexual activity, sexual inexperienced and loss of sexual interest of the partner.

What causes premature ejaculation?

For most cases of premature ejaculation the causes isn’t clear. With age and sexual experience, men often learn to delay orgasm. Premature ejaculation may occur with a new partner or it may happen only in certain sexual situations or if it has been a long time since the last ejaculation. According to doctors, premature ejaculation involves a complex interaction of the psychological and biological factors.

Psychological causes:

It includes various factors such as- poor body image, early sexual experiences, depression, sexual abuse, guilt feelings which increases your tendency to rush through sexual encounters and worrying about premature ejaculation. Other factors include relationship problems, erectile dysfunction and anxiety.

Biological causes:

It involves abnormal levels of brain chemicals called as neurotransmitters, inherited traits, abnormal hormone levels and inflammation and infection of the prostate or urethra.

What are the Signs and symptoms?

The main signs and symptoms of premature ejaculation include:

  • Decreased sexual pleasure due to poor control over ejaculation
  • Ejaculation which routinely occurs with little sexual stimulation and with little control
  • Feelings of guilt, frustration or embarrassment

Risk Factors and Complications

There are various factors which increases your risk of premature ejaculation such as stress and erectile dysfunction.

The complications due to premature ejaculation can cause problems in your personal life such as fertility problems and stress and relationship problems.

How is premature ejaculation diagnosed?

Your doctor will discuss about your medical and sexual history. He/She will perform a thorough physical exam. Your doctor may also want to talk to your partner. Since the premature ejaculation can have many causes, therefore your doctor may order lab tests to rule out any other medical conditions.

How is it treated?

In most cases, premature ejaculation gets better on its own over a period of time and treatment may not be required. Using distraction methods or practicing relaxation techniques may help to delay ejaculation. For some men, stopping or cutting down on the use of tobacco, alcohol, or illegal drugs may improve how well they can control ejaculation.

Other helpful techniques such as the “stop and start” method involves sexually stimulating the man until he feels that he is about the reach orgasm and stop the stimulation for around 30 seconds and then start again can help control premature ejaculation. Further, you can try the “squeeze method” which involves the sexually stimulating the man until he recognizes he is about to ejaculate and at this point, the man or his partner will gently squeeze the end of the penis for several seconds. Repeat the pattern until the man wants to ejaculate.

Often antidepressants and other selective serotonin reuptake inhibitors (SSRIs) are prescribed. However, they have side effects such as it will increase the time to reach ejaculation. You can apply a local anesthetic cream to the penis for reducing stimulation that will delay the ejaculation. For some men, using condom will have the same effect.

If all the above mentioned techniques do not heal you keep an erection, then opt for the medications used for erectile dysfunction or consult your doctor.

Your doctor may recommend you and your partner to practice certain techniques in order to help delay the ejaculation. For example, you may learn to identify and control the sensations leading up to ejaculation and may also learn to communicate with your partner to slow or stop stimulation. Try using a condom to reduce the sensations to the penis. Or you may try a different position such as lying on your back during the intercourse. Counseling or behavioral therapy may also help to reduce the anxiety related to premature ejaculation. Evaluation by a sex therapist, psychiatrist or psychologist may help some couples.

Involve your partner

If you are having problems with your sex life and are seeking treatment, it is recommended to involve your partner as much as possible. Communicating your concerns with your partner can help to resolve them which are essential on the long run. In certain cases, your partner may also have their own problems that contribute towards the problems with your sex life.

The best tip to deal with premature ejaculation is to focus on other sexual pleasures. This will help to decrease anxiety which in turn aids you to gain better control over your ejaculation.

Although premature ejaculation can cause anxiety and strain in a relationship, it is a treatable condition. Meanwhile, you may also consider exploring other ways in which you and your partner can connect with one another. In most cases, the man is able to learn how to control ejaculation through education and practising. Although there is no way to prevent this disorder, relaxation techniques can make it less likely to occur. Remember, one instance of premature ejaculation doesn’t mean you have a condition needing treatment. Hence it is significant to speak with your doctor if you consistently ejaculate before you want to.
